Robert Xiao 996fb1ff36 Avoid recreating ref'd types that already exist ...

Because TypesByReferenceIndex can be populated in two places
(Il2CppModel constructor and GetTypeFromVirtualAddress), we need to
avoid generating the same type multiple times.
