Move duplicated sanitizeIdentifier to Extensions
Also ensure that the identifier always starts with an underscore or
letter, which fixes issues with obfuscated identifiers.

Finish types implementation: vtables and classes.
Also fix a few minor bugs involving arrays and enums. Both should not
have __Class structures generated.
